The WV Board of Physical Therapy has the responsibility of investigating any wrong-doings of licensed Physical Therapists and Physical Therapy Assistants. We take any allegation of illegal or immoral behavior very seriously and investigate any complaints as quickly and thoroughly as possible. In the event that a PT/PTA has been found to have violated any of the Rules or Regulations pertaining to their profession, the Board will issue a penalty appropriate for the infraction. All disciplinary actions issued by the Board are a matter of public record.

Once the complaint form is submitted, it will become public information and may be disseminated as necessary in order to investigate and decide the alleged incident. We cannot guarantee that the information you submit will be kept confidential or that you will be able to remain anonymous.
